How it works
Your portrait is scaled to about one and a fifth megapixels and repainted at the lowest denoise of any page on this site, with a fresh seed each run. The instruction targets active spots, redness, and inflammation, and names four things to leave completely intact: pore structure, surface texture, moles and birthmarks, and the natural variation in skin color.
The negative prompt is aimed squarely at the standard failure — smoothed pores, blurred skin, a uniform plastic surface, a waxy sheen — because that is what almost every consumer tool in this category produces and it is what the low denoise exists to prevent.
Color, not scale
The design idea worth explaining is what distinguishes this from a smoothing filter.
Consider a spot and a pore side by side. In size they are comparable — both occupy a small handful of pixels at portrait resolution. Any process that identifies things by how small they are will treat them identically, which is exactly why smoothing removes both and leaves a surface with no detail in it.
But they are not similar in color. An active spot is markedly redder than the skin around it, often with a surrounding zone of milder redness. A pore is a slight shadow, essentially the same hue as the skin, only fractionally darker. Those are far apart in color terms even though they are close in size.
So the instruction is written around inflammation and redness rather than around small marks. That gives the model a signal that distinguishes the two, which is the difference between this page and the plastic result everyone recognizes.
What it cannot separate
Two situations sit outside that signal and it is better to expect them.
A raised spot under directional light casts a small shadow. That shadow is geometry, not color, and it does not read as inflammation. The redness can go while the bump remains as a faint tonal dimple, which looks odd precisely because it is halfway. Shooting under soft, frontal light removes most of the problem at source and is worth more than any setting here.
Post-inflammatory marks — the flat brown or purple patches that stay behind after a spot has healed — sit in between. They are a color difference, so the pass does affect them, but they are broader and less saturated than active inflammation, so it affects them less. Partial improvement is the honest expectation.
Check the texture, not the spots
The natural way to evaluate this page is to look for whether the spots are gone, and that is the wrong test, because they will be.
The real test is what happened to everything else. Open the output at one hundred percent, put the cursor over a clear area of cheek, and ask whether you can still see pores. Then look at the region a spot used to occupy and ask the same question. A good result has texture in both places. A bad one has a smooth patch exactly the shape of the thing that was removed, which is more noticeable than the original ever was.

Frequently asked questions
Why do spot removers make skin look plastic?
Because they usually work by scale rather than by color. A spot is a few pixels across and so is a pore, so a filter that smooths at that scale cannot tell them apart and removes both. What is left has the right color and no surface, which the eye recognizes immediately as not being skin.
Will it remove my moles?
It is instructed not to, and moles are named in both prompts. Look anyway, and not only for how the picture reads. If you have photographs that show your moles over time, they are a genuinely useful record, and quietly editing them out of every image removes the comparison you would want if one ever changed.
